Assistant Dean

The Office of Student Affairs is composed of the admission and registration departments; ojt department; housing, student activities and graduation departments; and the counseling and graduates follow-up department.

 

The office is responsible for monitoring student registration, student progress and advancement, graduation, counseling and all aspects of student life. It also coordinates student events, services and programs that support the academic mission of the College and responds to the needs of students.

 

The Office of Student Affairs provides career advising and in addition, the office provides ample trainings for students in their quest for career advancement.

Forms

Borrowing of Certificates Form

Used by all registered students in the college.

Change of Address Form

Used by all the students who have an acceptable reason for allowance increase.

Clearance Form

Used by all students who: 1) graduated from the college; 2) transfered from the college; 3) dismissed; 4) withdrawn from the college.

Exam Results Appeal Form

Used by students who want to appeal for their results in some courses after the results have been announced.

Excuse Form for Exam(s)

Used by all the students who don't appear for the Midterm or Final exam and have an acceptable excuse.

Postponement Form

Used by students who want to postpone their studies in any semester. Submission of the form doesn't mean it is accepted. The student has to follow up with the registration department to know the status.

Request for Duplicate Documents Form

Used by all students who lost their certificates like graduate certificates, or those students who lost their ID card. Students have to pay OR 5 for each document.

Student Activities Membership Form

Used by all students who are interested to obtain membership in student activities.

Transfer Form

Used by students who want to transfer from the college. Submission of the form doesn't mean it is accepted. The student has to follow up with the registration department to know the status.

Withdrawal Form

Used by all students who want to withdraw from the college.
